To recognize a person, who by his or her actions, has indicated exceptional
support of physical therapy.
CRITERIA FOR SELECTION:
Has supported the profession in the community.
Has contributed to the professionalism of physical
therapy.
Has portrayed the profession in a positive way
PROCEDURE FOR NOMINATION
Nominations may be submitted by any current member of
the association.
Nominations must be submitted in writing to include
Name, Contact information and a written statement of why the individual
is a worthy candidate. The written statement must include
Name
Signature
APTA Number of Nominator
Nominations should be sent to IPTA Executive
Director at office@ptidaho.org .It will be forwarded to the
nominating committee chair.
Completed nominations must be received by August 1st for
consideration that year.
Letters of support can be submitted from PT’s
,PTA’s, patients, or administration having knowledge of nominee’s
service record and character.
PROCEDURE FOR SELECTION:
All nominations will be sent to chair of the nomination
committee
List of nominees will be sent via email to all board
members who will review all information and make respective votes via
e-mail to nominating committee chair.
Results will then be sent to IPTA Executive Director so
the plaque or certificate can be ordered.
Award will presented at the IPTA fall meeting in
September.
